We are contracted as a "Preferred Provider" through Inclusa, which serves individuals in several counties in Wisconsin. Inclusa is a Management Care Organization (MCO) in the Family Care Program, a state-wide program that uses Medicare funding to service qualifying individuals. There are many other services and agencies that are in this community to serve the needs and requirements of the individuals in the Inclusa qualifying program. Contact Inclusa to find out more information or to obtain a list of the "Preferred Provider Network". This information can be obtained through their website or by contacting a community resource coordinator or health and wellness coordinator in the Inclusa organization.
Aging and Disability Resource Centers (ADRCs) are the first place to go to get accurate, unbiased information on all aspects of life related to aging or living with a disability. ADRCs are friendly, welcoming places where anyone - individuals, concerned families or friends, or professionals working with issues related to aging or disabilities - can go for information tailored to their situation. The ADRC provides information on broad range of programs and services, helps people understand the various long term care options available to them, helps people apply for programs and benefits, and serves as the access point for publicly-funded long term care. These services can be provided at the ADRC, via telephone, or through a home visit, whichever is more convenient to the individual seeking help.

The mission of Respite Care Association of Wisconsin is 'Promote, support, and expand quality statewide respite care across the lifespan."
RESPITE is the temporary relief for caregivers and families who are caring for people with disabilities or other special needs such as chronic or terminal illnesses or are at risk of abuse and neglect.
Respite Care Association of Wisconsin (RCAW) was organized in 1987 to support and advocate for quality systems of respite care for Wisconsin families.

The Wisconsin Department of Health Services (DHS) is the State of Wisconsin's website for assistance and available resources/services run by the State of Wisconsin. It also provides valuable information about legal rights, regulations and how to report individuals or services that you feel have wronged you or someone you know.
DHS connects individuals with the ways to get help as well as what is needed to qualify for government assistance. Visit www.dhs.wisconsin.gov for more information.
As with the State of Wisconsin, the United States Government has a department that oversees agencies, services and individuals that work in and are a part of our health care system. www.hhs.gov is the place to go for national level programs, agencies or to find information about the place to connect to in another state if a move or relocation is happening.
HHS.gov also offers resources and information for people doing research or seeking further information about available programs, how they work and how to qualify and apply for them. You can also find out about the latest national-level information pertaining to laws and declarations.
We work with individuals that are under numerous Management Care Organizations (MCO), or if they are under a self-management program (I.R.I.S). The individuals that we provide care for must be diagnosed with an intellectual/developmental disability and must be aged 18 or older. You can contact 4U LLC for more information or contact your designated MCO coordinator to find out the qualifications and the number of hours your individual can get authorization for.
